    Freud's paper on melancholy is perhaps one of his best, still relevant today (as opposed to hist "death instinct" theory for example, which nobody thinks is tenable). As for gratitude-talk - well...

    Sam Vaknin loves the idea of doing mere "Research Study" and vomit it as if he is doing some hardwork and so he must be excused for not doing healing just because he does some hardwork called researching and informing others what he studied. He will not help himself heal and risk where it takes him but will add ideas about narcissism over his buried feelings and act a bit cool as if a better-off narcissist than other non-informed loser-narcissist.

    Much human suffering is not illness. Illness involves a significant deviation from biologically built in norms. These norms are not decided by people voting, by peoples opinions, but by biology, physiology, Neuro physiology. When behaviour violates such norms, we say a person is sick. But calling a person mentally sick is like saying our economy is sick. In other words, mental illness is a metaphor. Unfortunately it is a metaphor that is widely taken literally. Not all behaviour I dislike is sickness.

    So is mental illness (mental pain/psychopathology) not an illness? Are you saying it’s only an illness if there is a biological component….hmmm so mental ill-health in your opinion is not ill health., is that right? Trying to remember this paper Yalom talked about, not his work, how an individual develops will power and how adverse childhood experience can affect the development of will power so it’s a myth to assume we are all operating from the same capacity or baseline when it comes to will power…of course that takes us to a broader philosophical discussion about free will itself and if it truly exists and to what degree…. Can we separate the mental and physiological…isn’t that how modern medicine keeps failing in terms of primary care…yes we are very advanced in some aspects but it doesn’t strike me as that simple. Really enjoy listening to you Dr Carveth.

    So you don't integrate Hyde. I remember integrating identities was part of the treatment plan in some MPD/DID therapies. What a mess that was. Satanic Ritual Abuse. Alien Abduction. Could Hyde be the result of the merging of a sadistic superego and a tortured/rebelious id? A kind of self-fulfilling hypochondriasis?

    Thank you very much for thus short talk. When someone has been numbing thier pain or trauma with addiction, sometimes when the addiction stops they don't have the ego strength to deal with these traumas and become suicidal. Do you have thoughts regarding this?

      @@eitanturkel1 yes, they need to have already been in intensive psychoanalytic therapy or full scale, psych analysis, so that when the withdrawal takes place, they have a deep attachment and a context in which to grieve well being “held” and “contained” in the therapeutic relationship

    While I think analysis is much more likely to result in long term sobriety, I do not for a second believe it’s necessary, as you seem to suggest. I think 12-step programs demonstrate this. They’re full of people still suffering their primary trauma but clinging to sobriety thru a rigid compulsion and cult-like support system.

    Interesting. My own impression, after working with substance addiction for some time, is that the picture of addiction is more complicated and defies efforts to come up with a single type of explanation. Our collective lack of success in treating substance addiction analytically suggests we should try to expand our theories (not to narrow them) by considering the interplay of the range of factors-- predisposing, precipitating, perpetuating, and protective -- in multiple spheres (e.g., genetic, biological, intrapsychic, cultural, etc.). However, our practice in the consulting room will be driven by specialty. The gold standard of addiction treatment will probably be something far more inter-disciplinary and well-integrated than today's standard of care.

      I love Don’s corpus of work and believe the psychoanalytic/psychodynamic approach has merit but who but the well heeled with a chronic addiction could afford the couch time necessary to affect the necessary object relation change in a world governed by big pharma and insurance “Spiritus Contra Spiritum”.       @@seymourtompkins I’m coming at this from the perspective of a patient with substance use issues (and also that of a Buddhist). I don’t think addiction is remotely mysterious. It’s simply an aggravated case of what most human beings spend their lives doing. We get fixated on an easy source of satisfaction because we’re too troubled to easily enjoy more everyday pleasures. I’ve really never understood the general attitude of confusion toward addiction.

    How can projection be a process of a one-body psychology when the very "body" is part of a systemic environment which first of all makes it possible that something like a projection, which results from the interplay between interpretations of patterns observed in the environment and the thereto belonging emotional-bodily response and the conceptual environment in one's inner psychic organisation? It is merely impossible to project anything when there was nothing dynamic to project it on. This is inherently a logical bias within the argumentation. Projective identification thereby is the successful operation when an external source manipulates your perception about the content of itself. It is a form of intrusion, which only does benefit the one investing into such behaviours, if consciously or not. Because Projective identification is the identification with the aggressor / intrusive force's perceptive angle. Why would one engage in such a pattern? Out of fear for example. The difference between Projection and Projective identification is that one time the intrusion is successful (identification), which goes on the cost of the person's coherence of identity. When you were getting angry about a patient approaching you this very way as described in the example, from a systemic angle, this would mean, that either the patient is right, which makes you angry for you tried to suppress anger for another situation or it is that way that the person is making use of the technique of projective identification, which then generates an anger response since this was an intrusion of your boundaries, which we naturally defend by getting angry when someone tries to push them. It could be as well that the person was serious with the question and was interested about your circumstances, maybe for the person felt a certain tension in your expression - while you then actually projected another intention into this very question, because of your own awareness that you perceive a tension within yourself, which you can't classify (or whatever other option). Logically, there are not any more reasonable options. Meaning, PI is a process that is a systemic phenomenon and can be used within these systemic environments to "exchange" identitarian perspectives in a dysfunctional way, for example for the preservation of a highly dysfunctional system-body, which has only no or a fragmented sense of identity. So, in no case any of both patterns were to classify as one-body process. There are factually not any one-body processes. Greetings, Gandalf

    Thank you. I think that the concept of Projective-identification is very relevant to understanding socio-cultural movements, especially much of what is referred to as the "identitarian left" these days when they cast accusations of 'unconscious' bias and 'racism' at people.

    Really appreciate this because I've been reading Klein/about Klein lately and every time projective identification came up it seemed like regular old projection and I was getting so baffled that I seemed to be misunderstanding the concept. Explaining how Bion helped to expand the concept was very clarifying; thank you.

    Thanks for a very helpful lecture/podcast. I really enjoyed it. I'm currently looking at some research by Perry and Giuseppe, and they dont mention externalisation in their overview. I didnt hear you mention that either. As a clinical psychologist working a lot with personality disorders, this is surprising to me. Isnt externalisation, or "blaming others" a central defence?     I believe that the contradiction Freud seems to be clinging to is just a matter of semantics. To be clear, determinism holds the position that every Y has had a preceding X; that every effect has had a cause. In this sense, we do not have freewill. This is not the same distinction being made when Freud suggests we “free or liberate” ourselves from harm. If we are to push against determinism logically we understand that “free” in this sense is not the idea of freewill we have set out to obtain; simply suggest that not every Y has a preceding X, that there are indeed truly “free” actions in this sense. Notice that this is not the “freewill” you are looking for. Taking a page from Sam Harris, this is just randomness; this is completely different than “freeing” yourself from persecution inside an assumed deterministic reality. We are free to make choices within the contexts of whatever box is holding or limiting our choices.

    Hallo Don! I was wondering, if the case of your father helping people after the car accident could be seen not only as supression, but as kind of splitting, which is situative and which we need in order to make decisions and take actions. And later on he came back into the lower level of excitation, the splitted came together and he reacted emotionally, now operating in depressive position?
